We welcome new patients to join the Practice who have recently moved into the Practice area which consists of BURNLEY, CLIVIGER and WORSTHORNE.

We are currently accepting new patients. On the right you will see instructions on how to register as a patient.

We are a paperlight practice, which means all your medical history is recorded on the computer.

We can assure patients of complete confidentiality. Your rights are also protected by the Data Protection Act, which the practice is registered to.

If you change circumstances, i.e. address, name or telephone number, it is important that you inform reception as soon as possible so that your details can be amended. Alternatively fill in the online form here.

How to Register

  1. Step 1

    Download and fill in our Practice Registration form.

  2. Step 2

    Download and fill in our Patient Questionnaire.

  3. Step 3

    Come into the Practice with the above forms filmed in and a form of photo id (such as a driving license or passport) along with confirmation of your address (such as a utility bill).
